News2 Score Calculator

NEWS2 aggregates six routinely-recorded vital signs — respiratory rate, oxygen saturation, supplemental oxygen use, temperature, systolic blood pressure, heart rate — plus a level-of-consciousness assessment into a single 0-20 severity score, precisely so that ward nurses can spot early physiological deterioration before it becomes an overt emergency. Each parameter is scored 0-3 based on how far it deviates from a normal range, and unlike a simple average, any single extremely abnormal parameter (a 'red' score of 3 in one category) triggers an urgent review even if the aggregate total is otherwise low, since a severely deranged single system can be masked by normal values elsewhere. The 2017 NEWS2 revision added a second oxygen saturation scale specifically for patients with hypercapnic respiratory failure (chiefly COPD patients on physician-directed target saturations of 88-92%), because the original NEWS scale penalized these patients for saturations that were, for them, actually appropriate and safe. Aggregate score bands map directly to an escalation protocol: low scores prompt routine monitoring, medium scores prompt an urgent clinical review, and high scores (7 or above, or any single red parameter) prompt an emergency response, typically involving a critical care outreach team — turning a set of numbers into a standardized, auditable trigger for calling for help.

The Royal College of Physicians in the UK developed the original National Early Warning Score in 2012, responding to years of evidence that ward-based early-warning systems varied enormously between hospitals — some using entirely different scoring weightings and thresholds — making it impossible for staff moving between hospitals or regions to interpret a colleague's handover consistently, and complicating multi-site clinical research on deterioration and rapid-response outcomes. Following feedback that the original scale's single oxygen-saturation scoring band disadvantaged patients with chronic hypercapnic respiratory failure who are managed with deliberately lower target saturations, the RCP convened a further working group and published the revised NEWS2 in December 2017, adding the second oxygen-saturation scale and refining new-onset confusion criteria. NHS England subsequently mandated NEWS2 use across acute hospital trusts, and the score has since been adopted internationally as one of the standard aggregate early-warning tools referenced in sepsis-recognition and rapid-response-team protocols.
