How does the TIMI score guide treatment decisions in UA/NSTEMI?
A TIMI score of 0-2 (low risk, ~5%) can be managed conservatively with anti-ischemic therapy and early discharge with outpatient follow-up. Scores of 3-4 (moderate risk, ~13-20%) typically warrant in-hospital monitoring and early angiography within 24-72 hours. Scores of 5-7 (high risk, >26%) generally benefit from urgent invasive strategy (angiography within 24 hours) and more aggressive antiplatelet therapy.
What is the significance of the aspirin use criterion in the TIMI score?
Aspirin use within the 7 days prior to presentation is paradoxically a risk factor in the TIMI score because it identifies patients who were already being treated for cardiovascular disease and still developed an acute coronary syndrome. This suggests more severe or refractory disease, not that aspirin itself is harmful. Similarly, it may indicate aspirin resistance or medication non-compliance in a high-risk population.
How does TIMI compare to GRACE and HEART scores for ACS risk stratification?
TIMI is simple and quick to calculate at bedside. GRACE uses continuous variables (age, heart rate, blood pressure, creatinine) and provides more precise risk discrimination but requires a calculator. The HEART score combines history, ECG changes, age, risk factors, and troponin specifically for chest pain evaluation in the ED and has better discrimination for 30-day outcomes than TIMI in some studies.
