SCORAD folds three different kinds of information about atopic dermatitis into one number using the formula A/5 + 7B/2 + C. A is the extent of affected skin as a percentage, estimated by the rule of nines, contributing a maximum of 20. B is the intensity term, the sum of six signs each graded 0 to 3 at a representative lesion - erythema, edema or papulation, oozing or crusting, excoriation, lichenification, and dryness - giving 0 to 18, which the multiplier of 3.5 expands to a maximum of 63. C is the subjective term, pruritus plus sleep loss each rated 0 to 10 on a visual analogue scale over the past three days, contributing up to 20. Maximum total is 103, with under 25 read as mild, 25 to 50 moderate and above 50 severe, a threshold often used to justify systemic therapy such as dupilumab or ciclosporin. The weighting is a deliberate judgement: intensity accounts for roughly 61 percent of the possible score, extent under 20 percent. Two details are commonly mishandled. Dryness must be graded on uninvolved skin, not on a plaque, and the intensity signs are scored at a single representative site, so the result is sensitive to which lesion the assessor picks. The subjective component needs a patient of about seven years or older, or a caregiver proxy that introduces its own bias, which is why Objective SCORAD, dropping C entirely, is often preferred in trials. Infant proportions also differ, the head accounting for closer to 17 percent of surface area.

By the late 1980s eczema research had a comparability crisis. Numerous severity scales were in circulation, including the Rajka and Langeland grading and the Costa and Leicester indices, each measuring somewhat different things, which meant trials of the same treatment could not be pooled or compared. The European Task Force on Atopic Dermatitis, a working group convened under the European Academy of Dermatology and Venereology, was formed to impose a common standard. The task force published the SCORAD index in the journal Dermatology in 1993 under the title Severity scoring of atopic dermatitis: the SCORAD index. Its method was consensus rather than statistical derivation: a workshop selected the items, weights were assigned by agreement, and a photographic reference atlas was produced so that intensity grades meant the same thing in different clinics. Inter-observer reliability was then tested across multiple European centres. In 1997 the group described Objective SCORAD, removing the patient-reported term to eliminate a major source of variability in trials. SCORAD became and largely remains the dominant clinician-administered eczema instrument in European practice. The irony is that fragmentation returned: the Harmonising Outcome Measures for Eczema initiative, working through consensus meetings from 2011 onward, recommended EASI as the core measure of clinical signs and the Patient Oriented Eczema Measure for symptoms, addressing the same problem SCORAD had been built to solve two decades earlier.
