The Dermatology Life Quality Index measures how much a skin condition has disrupted a person's life over the preceding seven days, deliberately capturing something clinical severity scores cannot. Ten questions span six domains: symptoms and feelings in questions 1 and 2, daily activities in 3 and 4, leisure in 5 and 6, work or school in 7, personal relationships in 8 and 9, and burden of treatment in question 10. Each answer scores 0 for not at all, 1 for a little, 2 for a lot and 3 for very much, giving a total from 0 to 30. Conventional bands read 0 to 1 as no effect, 2 to 5 small, 6 to 10 moderate, 11 to 20 very large and 21 to 30 extremely large. A score of 10 or above alongside a PASI of 10 or above is the eligibility rule many health systems apply to biologic therapy in psoriasis, and the minimum clinically important difference is generally taken as about 4 points. The short recall window is both the instrument's strength and its main limitation, since a flaring week and a quiet week in the same patient can differ by ten points. Question 7 and question 9 offer a not relevant option that scores zero, which systematically deflates totals in retired, unemployed or unpartnered respondents and complicates comparison across groups. The index also shows floor effects in mild disease and was not designed for children, for whom the separate Children's DLQI exists.

The DLQI was developed by Andrew Y. Finlay and Gul Karim Khan at the University of Wales College of Medicine in Cardiff and published in Clinical and Experimental Dermatology in 1994. It was the first quality of life instrument specific to dermatology. Rather than writing questions from theory, the Cardiff group interviewed around 120 patients with a range of skin diseases and asked what problems their condition actually caused, then selected the items that patients raised most often. A deliberate design constraint was brevity: the questionnaire had to be completable in roughly two minutes without assistance, because anything longer would not survive a routine clinic. The instrument spread quickly because that constraint made it usable in trials as well as practice, and because dermatology had until then relied on physician-graded signs alone. The Cardiff group extended the family with the Children's Dermatology Life Quality Index in 1995 and later instruments measuring impact on family members. Descriptive severity bands for the score were established by Hongbo and colleagues in the Journal of Investigative Dermatology in 2005, which gave the raw totals a clinical meaning they had previously lacked. The DLQI has since been translated into more than ninety languages, embedded in reimbursement criteria by bodies including the United Kingdom National Institute for Health and Care Excellence, and used as a supporting endpoint in regulatory submissions for psoriasis and atopic dermatitis therapies.
