In many guidelines, DLQI ≥10 (plus PASI ≥10) qualifies for biologic therapy. DLQI ≥10 indicates a very large impact on quality of life.
What skin conditions is the DLQI validated for?
The Dermatology Life Quality Index was originally developed for adults with any dermatological condition and has been validated for psoriasis, atopic dermatitis (eczema), acne, urticaria (hives), alopecia areata, vitiligo, and hidradenitis suppurativa. It has been translated into over 90 languages and used in more than 80 countries. The DLQI is a standard outcome measure in clinical trials and is required by many national health authorities when evaluating new therapies for reimbursement decisions.
How do the 10 DLQI questions relate to different aspects of life quality?
The 10 DLQI questions cover six domains: Symptoms and feelings (questions 1-2) covering itching, soreness, and embarrassment; Daily activities (questions 3-4) covering shopping, gardening, home, and clothing choice; Leisure (questions 5-6) covering social and sporting activities; Work and school (question 7); Personal relationships (questions 8-9) covering partner, close friends, and sexual difficulties; and Treatment (question 10) covering how much the treatment itself causes problems. Each question is scored 0 to 3, giving a maximum total of 30.
What is a clinically meaningful change in DLQI score?
The minimum clinically important difference (MCID) for the DLQI is generally considered to be a change of 4 points for psoriasis and approximately 3-4 points for atopic dermatitis. This means a treatment must reduce the DLQI by at least 4 points to be considered as providing meaningful patient benefit beyond placebo or natural disease variation. Modern biologic treatments typically achieve reductions of 8-12 points from baseline, representing very large improvements in quality of life for patients with severe disease.
