How does smoking affect cardiovascular disease risk?
Smoking is one of the most powerful modifiable risk factors for cardiovascular disease. It damages the inner lining of blood vessels (endothelium), promotes inflammation, reduces HDL cholesterol, increases LDL oxidation, raises blood pressure, and makes blood more likely to clot. Who should get a formal cardiovascular risk assessment instead of an estimate?
Most major guidelines (ACC/AHA, WHO) recommend a formal 10-year CVD risk calculation for adults aged 40 to 75 without existing heart disease, using an actual fasting or non-fasting lipid panel and a clinic blood pressure reading rather than home estimates. If your estimated risk here comes back moderate or high, or if you have diabetes, a family history of early heart disease, chronic kidney disease, or an LDL cholesterol above 190 mg/dL, you fall into a category where guidelines recommend risk-lowering treatment discussions regardless of the calculated percentage, so bring these numbers to a clinician rather than relying on the estimate alone.
