Methadone is a special case: its conversion factor is nonlinear and increases sharply at higher existing daily doses due to unpredictable accumulation and a long, variable half-life, so it requires a separate stepped conversion table rather than a single multiplier; transdermal fentanyl similarly uses its own mcg/hr-to-MME conversion because it is absorbed continuously through skin rather than dosed episodically. The resulting daily MME total is not a precision pharmacokinetic measurement — it is a population-level risk-stratification tool. CDC guidance flags 50 MME/day as a threshold where prescribers should reassess the benefit-risk balance, and 90 MME/day or higher as associated with a substantially elevated risk of opioid-related overdose death in observational data, prompting avoidance or careful justification with additional safeguards (naloxone co-prescribing, more frequent monitoring) above that level. MME does not account for individual tolerance, so a stable long-term patient on a high dose is clinically different from an opioid-naive patient started at the same MME.
