Lifestyle dominates lifespan: indoor cats commonly reach twelve to eighteen years, while free-roaming cats face vehicle trauma, predation and infectious disease including feline leukaemia virus and feline immunodeficiency virus. Breed, body condition and neuter status modify the picture further, and obesity is a documented risk factor for diabetes and reduced longevity. For a cat of unknown age, veterinarians estimate from dental wear and tartar, lens changes such as nuclear sclerosis, and coat and muscle condition, all of which yield a range rather than a number.

The idea of converting pet years into human years is far older than any evidence supporting it. The habitual multiply-by-seven rule circulated widely through the twentieth century, apparently as a crude comparison of average human and dog lifespans, and was applied to cats by simple analogy despite the two species maturing at very different rates. Veterinary writers criticized it for decades on the obvious ground that it makes a one-year-old cat seven, an absurd figure for an animal that can already reproduce and has finished growing. The staged scheme used today was formalized by professional bodies rather than by folklore. The Feline Advisory Bureau in the United Kingdom, now International Cat Care, promoted a life-stage framework of kitten, junior, prime, mature, senior and geriatric, and the American Association of Feline Practitioners together with the American Animal Hospital Association published Feline Life Stage Guidelines in 2010 that tied specific preventive care recommendations to each band. The two organizations issued a revised version in 2021 that simplified the categories and stressed individual assessment over rigid age cut-offs. Documented extreme longevity anchors the upper end of the scale: Creme Puff, a cat from Austin, Texas, was recognized by Guinness World Records as having lived 38 years, from 1967 to 2005.
